PROM REQUIREMENTS:
Everyone attending the prom on May 21st is required to have a negative Covid test result 48-72 hrs prior to the prom. Cab is offering free testing to all attendees on May 18th from 2pm – 5 pm in the auditorium. Registration is suggested but walk-ins are accepted. The link to register is below.

1) Click on the E-Req link provided above
2) Fill in as much patient info as possible (PLEASE NOTE THAT ONLY PCR TESTING IS AVAILABLE FOR THIS EVENT)
3) Select the CORRECT COLLECTION DATE FOR YOUR EVENT (*** Please note that although there is a slot to select an appointment time, this event is walk-up only…no appointments times!)
4) Click “Create E-Req”
5) Print out E-Req
6) BRING YOUR PAPER E-REQ TO THE TESTING SITE IN ORDER TO SPEED UP YOUR PROCESS!!!!
