Hello,
I am reaching out to schedule the COVID vaccine at the SBHC. The next date is Friday 10/22 due to stock timing. We are only vaccinating age 12 and up at this time. Please have your parent/guardian do the following items to get your vaccine as soon as possible.
- If not a member of the Wellness Center, fill out our e-registration form for the SBHC in the link below. I have attached the information about our center as well. It asks for insurance because we process the vaccines through it, but we do not have any co-pays or extra fees. Your registration is good for all of high school and for other visits as well.

- The COVID consent PDF is attached- please complete and sign and either email back or have your child bring to the appointment Friday 10/22.

- Call the center at 302-651-2100 or email back a time on Friday 10/22 to schedule your child to get the vaccine. We can schedule 10am-2pm that day. Some like to schedule during their activity period or at the end of lunch. The visit will take about 30 minutes from start to finish. The student has to wait 15 minutes before going back to class to make sure there is no adverse reaction.
